This standardizes selection tools in SELECTION_TOOL class to be able to
handle a long-click as asking for a non-heuristic disambiguation menu.

The current and previous net highlight state are now stored and can be toggled
on or off with an action (no hotkey by default).
Highlight is now treated as a persistent state rather than a tool that is entered,
meaning there is no longer a special tool to enter in order to highlight nets.
Highlighting can be controlled while any other tool is active using the hotkeys
or context menu on a net object.
Tweak the icons for highlight net and layer display mode to better disambiguate them.

The last remaining threaded operation in dynamic ratsnest was the
recalculation of the dynamic connectivity map. Because we do not
require any of the extra features of the connectivity map, we can get
away with a lightweight move of the anchors to update the ratsnest. The
resulting connectivity tree is not valid but it is not needed for the
ratsnest, which only needs a list of nets/anchors.

avoid potential performance issues when the dialog is still listening to
the board updates and refreshing its data and view structures.
it seems useful to keep the settings of the dialog across dialog
open-close cycles.
Also fixes some interactivity issues in the microwave tools around
single-click tools that have initial properties dialogs. Those
need to act kind of like a hybrid between single-click and two-click
tools.

This adds the microwave tools to GAL in Pcbnew as a new tool:
MICROWAVE_TOOL.
Some new preview items are introduced to support this:
* TWO_POINT_GEOM_MANAGER for managing construction of two-point
geometries, such as that used to construct the inductor.
* CENTRELINE_RECT_ITEM a preview item to draw ractangle with a given
aspect ratio along a line (specified using a TWO_POINT_GEOM_MANAGER)
PCB_TOOL gets a generic event loops which should be useful for more
than just microwave tools:
* doInteractiveItemPlacement() - handles event loops that wait for a
click, create an item on click and then allow moving it
around/flipping, etc.

The apply settings and push settings options now take account of the
validity of the global pad settings when deciding whether to show or
not.
Global pad push now requires a source pad and doesn't push from the
global pad setting when there is no selection.
The whole submenu is hidden when there are no enabled items.

Import becomes apply and export becomes copy.
The term "push" to apply settings globally has not been renamed.
Swap the order of the copy and apply in the context menu.
This implements the pad import/export to the board's master pad setting
in the GAL canvases.
Implemented as a new GAL tool: PAD_TOOL.
It uses the same dialog, which has been split out into its own files in
pcbnew/dialogs, rather than along with frame methods in
pcbnew/globaleditpad.cpp.

face normals distorting the vertex normal; this will be fixed by
